give cash refunds for patrons who lost their money in a coin
tower. Patrons who lost money using their Plus Account are
not eligible to receive a cash refund. Any coin tower
refunds must also be picked up at 2-32 W.T. Young
Library at a mutually beneficial time. If it is urgent,
please call 859-257-9376 instead of completing this
form. You will also receive an e-mail copy of this
request based on your E-mail below, do not CC yourself.

completing this form, copies will be sent to your Consultant E-mail
address, the Patron's E-mail Address and any CC parties in addition to
those responsible for handling the refund.
Note: If any
patron has lost money using the Plus Account, please
call the
IKON Office at 859-257-9376 before filling out a refund request
form during our regular business hours.
We will be able to print your job off for free and this helps us to
prevent these problems from reoccurring.
